Guiding the Journey To Success: Best Practices in Case Management

All Vibrant Training sessions for DYCD-funded program staff will be facilitated online using Zoom throughout the end of June. We appreciate your patience with this process, as it is new technology for many of us.
Following your registration on EventBrite, you will receive the Zoom registration link. You will need to register for the session on Zoom prior to the start of the training to receive the link in a confirmation email to attend the Zoom training.
After attending the session, you will receive a certificate of attendance via email along with the materials shared in the session within 72 hours.
Target Audience: This workshop is intended for all caseworkers and staff within DYCD-funded programs that provide case management services.
Have you ever tried to get somewhere but forgot the roadmap? Getting from Point A to Point B requires guidance and support, and when working with clients to achieve outcomes, you are often the GPS for clients’ journey to reach their final destination.
Effective case management begins with a strength-based assessment that provides a holistic picture of the client and his/her needs for participating in your program. This foundational assessment is crucial, and acts as the basis for all on-going work with the client. From here, setting clear and achievable goals with the client, outlining measurable action steps, and documenting progress are the on-going guidance required to support successful outcomes. This virtual interactive workshop will use case studies, videos, group discussions and practical tools to enhance your effective case management skills.
